Build Contact Information Question

Q. How do I build a question for requesting contact information?


A. For this type of question, use a Grid Question with question type Alpha Text Boxes. For the row labels, enter the contact information labels. For example:

Row Labels

Name
Address
Email
Phone

Since you do not really need a column label, enter in a double colon. For example:

Column Labels:
____________

Using a double colon forces the application to not display a row header (the first row). To adjust your column sizing, use Format Answer Options.
